Job Description

Our Internal Medicine practice offers an endocrinology specialty clinic, which treats disorders such as diabetes, thyroid disorders, and other endocrinology related disease processes. Additionally, the Internal Medicine practice cares for patients experiencing chronic diseases such as heart disease, high blood pressure, medication management, and offers a focus on overall patient health and wellness. The primary purpose of this position is to work collaboratively as an active member of the interdisciplinary team to provide patient centered care across the continuum. In this role the goals include to provide an excellent patient experience, high quality outcomes, and improved productivity for the team. This position has the opportunity to work from home one day per week. Specific responsibilities include collaborating with consulting providers including endocrinologists, nutrition, and pharmacists; identifying opportunities for nurse led visits such as annual wellness visits, new patient intakes, and acute visits; supporting coordination of team-based care with CNAs, CMAs, LPNs, and other RNs within the practice; participating in the on-going design and implementation of nurse led protocols and co-visits; providing panel management support to identify quality care gaps and coordinate outreach; facilitating nursing phone triage to assist with management of urgent and emergent patient care situations; providing care management support such as transitions of care calls and hospital follow-ups within clinic; performing phlebotomy, clia waived lab tests, processing specimens, and starting IVs as needed; serving as a preceptor for new clinical staff; and participating in process improvement of new and existing workflows. The role requires excellent communication, decision making, healthcare knowledge, interpersonal skills, adherence to organizational values, and problem solving abilities. Computer skills including Microsoft Office and electronic health record documentation are required. Spanish speaking skills are preferred. Physical demands include light to moderate energy level with lifting and carrying 25-35 lbs and pushing/pulling 50-100 lbs. Minimum qualifications include a Bachelor’s Degree in Nursing, current unencumbered North Carolina nursing license, and current CPR certification. Preferred qualifications include two years relevant experience or comparable combined experience and/or education, one to two years of nursing experience in an ambulatory setting, CDE preferred, and endocrinology experience preferred. The schedule is Monday – Friday, 8:00 am to 5:00 pm, 40 hours per week with occasional weekend, holiday, or evening coverage. Compensation is $33.32/hour, full time with full benefits available.
